How to Respond to a Request for Customer Information

If you own or lease land within an irrigation project where BIA assesses fees and collects monies to administer, operate, maintain, and rehabilitate project facilities this information is being collected from individuals and organizations doing business with the government as required under the Debt Collection Improvement Act of 1996 (DCIA). The principal purpose for collecting this information is for billing, including collections, proper payment application and debt management actions. Information collected is used by the Bureau of Indian Affairs (BIA) for servicing the account.

What You Need

You will need the Request for Customer Information Form, and the mailing address for your local project office. Contact your local irrigation project for details on submitting the form.
